How To Get Philhealth ID Number and Card If You Lost or Misplaced It

Did you lose your Philhealth Card and you just can't remember your Philhealth Identification Number? Is it nowhere to be found in your email inbox or even on printed documents?

Well, I have great news for you.

I recently discovered the official and easy way to get hold of your PhilHealth ID Number. I've tried it out myself so I know that it works.

Here's the official email that I received from PhilHealth giving me my ID number.

Let me share the process that I used through this step by step guide. I have to note, however, that this is not for those who need to get the information immediately as you will have to wait several days to get a reply.

1. Open your email account.

2. Compose and send an email to actioncenter@philhealth.gov.ph

3. On the subject, type "Request for Philhealth ID Number".

4. Follow this text format for the email itself:

Hello PhilHealth! Good day! 

I would like to request for my PhilHealth ID number as I can't quite find my card.

I'd appreciate any help you can give me.

Here are my details:

Name: _____________

Birthday: Month, Day, Year

Place of Birth: _______________

Address: ____________________

BIR TIN: __________________

I attached my ID for your reference.

Thank you!

4. Attach an image of your government-issued ID (Passport, Voter's ID, PhilSys, Driver's License, etc.)  to the email.

5. Press Send.

6. Wait a few days to get an email reply from PhilHealth.

7. In my case, it took PhilHealth five days to reply to my email with my Identification Number. Be patient.

Where To Get PhilHealth ID Card?

After you get your PhilHealth ID number, take note of it and don't lose it again as you will use it for all your transactions with the agency. You may request for a copy of your PhilHealth ID card at any PhilHealth Local Health Insurance Office (LHIO) near you. Sadly, currently, there's no way to request for or order a PhilHealth ID online. Just use Google to search for those centers. Bring a valid government issued ID/s for verification purposes.

Additionally, you can now access the Member Portal in the PhilHealth website at memberinquiry.philhealth.gov.ph/member/ Through this portal, you may view your member information and premium contribution as well as printing/downloading of Member Data Record (MDR). Simply register and create an account using your unique PhilHealth Identification Number (PIN) and personal information.
